Follow these steps to sign up for a myChart account.

• Enter your personal information.
• Verify your identity.
• Choose a username and password.
• The account you are creating will provide you access to your own health record information. If your need is to access someone else’s health records who is over 12 years old, please reach out to that person to send an invitation from their account (found in the Sharing Hub). If under 12 years old, contact that patient’s office to complete the forms.
